Please submit the details below to receive your reports. You can opt for either digital copy via Email or printed reports. In case of a digital copy printed reports you can visit Tabba Heart Institute or any collection point to get the reports.
*Reports request will be entertained after 1 week of discharge or 3 days after the request is made whichever date is higher*
Please select how would you like to receive the reports. For Printed Reports digital copies, you will have to visit the nearest collection unit or main hospital.
