Patient Safety & Quality Assurance Mission:

PS&QA Department is inspired to be foremost the recognized leader in improving quality and patient safety for THI by continuously maintaining the highest standards and to become the module of distinction nationally and internationally.

PS&QA Vision:

The PS&QA department is committed to improve the quality of healthcare provided through THI by promoting a collaborative quality culture, teamwork spirit and quality training as illustrated in the Quality management program.

The purpose of the THI Quality and Patient Safety  Plan is:

  • To provide a framework for defining quality and performance improvement opportunities, implementing actions, and evaluating results.
  • To facilitate communication, reporting, and documentation of all performance improvement and patient safety activities to staff and administration.
  • To document an organized, coordinated and clear manner of identifying risk   factors to the THI, to promote and support development of practices aimed at minimizing the adverse effects of loss and to reduce, modify, eliminate and control conditions that may cause loss.
  • Educate and involve staff in quality improvement initiatives.

The Patient Safety & Quality Assurance Department (PS&QAD) will ensure that appropriate processes of quality improvement activities are in place in all the clinical and non clinical departments. Identify, define, standardize and monitor hospital-wide clinical / non clinical quality indicators. All hospital indicators should be compiled and major indicators should be incorporated in the dashboards. Compare the existing trends against the defined standards and if needed then recommend appropriate corrective actions to review and revise the standards. Submit monthly/ or according to the schedule minutes of the meeting to the hospital leadership listing activities, identified problems with proposed solutions and recommendations and any alternative options.

 

The Department of PS&QAD major functions consist of:

  • Risk Management
  • Data Management
  • Quality Improvement
  • Accreditation

Evaluation of system compliance included a review of the information systems assessment, focusing on the processing of encounter data, patient data, and provider data. Additionally, the review evaluated the processes used to collect and calculate the performance indicators, including accurate numerator and denominator identification, and algorithmic compliance (which evaluated whether rate calculations were performed correctly, all data were combined appropriately, and numerator events were counted accurately). 

Quality Indicator monitoring providing a framework for quality will facilitate consistent implementation of quality improvement processes in every day. The tools provided and support current and future quality improvement efforts by providing system-level leadership in defining characteristics of quality in the system. The characteristics promote strategic decision-making and resource allocations to focus attention on the development of concentrated efforts to improve quality and ultimately improve patient health outcomes. The Quality Indicator captures an aspect of quality that is widely regarded as important and subject to healthcare provider. Consensual validity expands face validity beyond one person to the opinion of a Quality Assurance Representatives.
